01 About VateMed Mobile IV

VateMed Mobile IV, LLC is a limited liability company registered in the State of Georgia, founded by a registered nurse and preparing to provide mobile IV hydration and wellness infusion services in Metro Atlanta. The business is in pre-launch: entity formation is complete or in progress, and operational readiness (Medical Director, clinical protocols, insurance, licensing) is being assembled.

02 What this website currently does

Today, this website is an informational and interest-signup site. It does not book, schedule, confirm, deliver, or bill for any medical service. It has one primary interactive feature: a "Request a visit" form that lets interested individuals let us know they'd like to hear from us when services launch.

03 What we collect through the form

When you submit our "Request a visit" form, we collect the following:

• Your full name
• Phone number
• Email address
• ZIP code
• The type of service you're interested in
• Preferred date and time window
• Any notes or context you choose to share in the "anything we should know" field

We ask that you do not include detailed medical history, diagnoses, medications, or protected health information in the notes field at this time. During pre-launch, this data is not being collected or handled as protected health information under HIPAA. Full HIPAA-compliant intake will begin only when services launch.

04 How your information is used

Information you submit through the form is used only to:

• Contact you when VateMed's services become available in your area
• Answer questions you specifically ask
• Understand general demand and service interest as we plan our launch

We do not sell your information. We do not share it with advertisers, marketers, data brokers, or unrelated third parties. We do not currently use it to target advertising to you.

05 Where your information is stored

Form submissions are processed and stored by Formspree, our third-party form processor. Formspree is a standard, widely-used service; you can review their privacy practices at formspree.io/legal/privacy-policy.

Our website itself is hosted by Netlify. Netlify may collect standard server-side logs (such as IP addresses and browser types) associated with visits to this site as part of normal hosting operations. You can review their practices at netlify.com/privacy.

06 Cookies and tracking

This site does not currently use analytics cookies, advertising cookies, or third-party tracking pixels. If we add analytics in the future — for example, to understand which pages visitors find useful — we will update this notice and publish a full Privacy Policy before doing so.

07 Your rights and choices

You can, at any time:

• Ask what information we have collected about you
• Ask us to correct any inaccurate information
• Ask us to delete your submission from our records
• Ask us not to contact you further

To make any of these requests, please email us using the contact block below. We will respond within a reasonable time.

08 What is still to come before launch

Before VateMed begins actually providing services, we will publish:

• A full Privacy Policy compliant with applicable state privacy laws, describing how client information is collected, stored, used, and protected once services are live.
• Terms of Service governing use of this website and, separately, terms governing the client–provider relationship for booked visits.
• A HIPAA Notice of Privacy Practices as required by federal law once VateMed becomes a covered entity handling protected health information.
• Clinical service disclaimers, informed consent language, and other pre-visit documentation consistent with the Georgia Composite Medical Board's May 2026 position statement on IV therapy.

All of these documents will be finalized in consultation with a licensed Georgia healthcare attorney before services begin.

09 Changes to this notice

We may update this notice as VateMed's pre-launch progresses — for example, if we add analytics, integrate a new tool, or approach launch. When we do, we'll update the "last updated" line at the top of this page. This notice will be replaced entirely by our full Privacy Policy and related documents at the time services launch.
